Both applicants must be present to complete the license unless there are extenuating circumstances. The Clerk or Judge can make the determination.
If applicants are under 16 years of age, the Judge’s permission must be obtained before the license is issued. They must file a civil petition.
If applicants are between 16 and 18 years old, the application must be signed by a parent or a notarized letter will suffice.
The fee is $60.00.
Both applicants must provide proof of identification such as a Driver's License, Birth Certificate, or Government Issued ID card.
The following information is required from the applicants:
- Social Security Numbers
- Mother’s and Father’s name and birth state
- Mother’s maiden name
- Both applicant’s birth date and birth state
- Number of this marriage (first, second)
- If divorced, what year and what state
